In consideration for participating in the above-referenced program / activity sponsored by the Recreation Department of the Town of Bolton, I hereby waive and release the Town of Bolton, its agents, officers and employees, whether paid or voluntary, from and against all claims, suits, actions, damages, liabilities, costs, expenses, and/or judgments, including attorneys fees and court costs, which may arise from my participation in the above – referenced program / activity or any illness or injury resuming there from, either directly or incidentally. I hereby represent that I understand and am familiar with the nature and type of activities in which I / my child will participate as part of the above-referenced program/activity. I further represent that my child is in good physical and mental health and that I am unaware of any physical or other health condition that would affect their ability to participate in Bolton Recreation Department activities. I give permission for all program volunteers and employees to provide emergency first aid or obtain emergency medical treatment for myself or my child in the event that an injury occurs. I agree to abide by the Recreation Department Policies with program registrations. I understand participants may be photographed and used in program publicity.

Player and Parent Code of Conduct– Any person who is using alcohol or illicit drugs and/or appears to be intoxicated or under the influence, or who is flagrantly rude, attempts to intimidate, verbal abuse or uses vulgarity or profane language/gestures with an official, coach, player or recreation staff will be asked to leave the game/program immediately. Any person who commits the above offenses a second time will be banned from the program for the remained of the season.
